Your current I-20 form from previous institution
Clear, legible copies of passport identification and visa pages
Required for F-2 dependents accompanying the student
Only required if status was changed within the USA
Any previous extension approvals if applicable
Entry record from initial arrival to United States
Complete all sections and provide required signatures
If using a sponsor for financial support
Photo identification of financial sponsor
If you had your status changed in USA, your Bank Statement needs to show the sum of:
